Demographics Survey and Amazon $25 Gift Certificate Giveaway

There are two parts to this survey so that those who take the first part can remain anonymous.  The results from each survey will get organized into TWO DIFFERENT spreadsheets.  That's why there are two different submit buttons.

The first part asks for demographic info so that I can get some idea about the kind of folks visiting Moana Saves and present this info to potential advertisers and sponsors (and double the fun around here~!).

The second part of the survey asks for your name and email address--again this information does not show up on the same spreadsheet with the demographic survey. One winner for the Amazon $25 gift card will be randomly chosen from those filling out the second part of the survey. The small print about the Amazon giveaway follows the second part of the survey (titled Amazon $25 Gift Certificate Giveaway).





Only one entry per person/email/household/IPaddress for this giveaway.  He/she must be 18+ years old and reside in the U.S.  This giveaway ends on Monday November 15th at 11:59 PM HST (Hawaii Standard Time).  One winner will be chosen randomly via Random.org.  I will notify the winner via email and he/she must respond to claim the prize within 48 hours or I will draw another winner.
